  Andras Domjan Unit Assessment record Unit Title: Introduction to communicate in health, social care or Childrens and young people settings Unit number: 4222-201 Unit Level: 2 Outcome 1: Understand why communication is important in adult social care settings 1.1   Identify different reasons why people communicate Different reasons why people communicate is to socialise with friends and families and to express their feelings and opinions, also to help make friends. 1.2   Explain how effective communication affects all aspects of working in adult social care settings Effective communication effects all aspects of my own work because it allows me to know a person’s  background from family and friends to the individuals themselves. It also allows me to learn of the i ndividual’s condition, whilst I have been off shift. This is important because a client maybe on new medication and I could just carry on with what I know and not check. It supports the smooth running of the establishment 1.3   Explain why it is important to observe an individual’s  reactions when communicating with them It is important to observe an individuals reaction when communication with them because someone’s reactions is away of discovering their real feelings towards a certain subject. As often their facial expressions and body language speak more than their words.   Outcome 3  –   Understand how to reduce barriers to communication. 3.1. Identify barriers of communication - Hearing impairments -Welsh language - Languages   -Learning difficulties -Vision impairments 3.4. Identify sources of information and support to enable more effective communication The best ways to find the more effective communication are: -   To do your own research into the particular ways of communicating and perhaps looking into local support group’s  which would help the individual break down barriers of communication. -   Other health professionals -   I can get help from my supervisors other care workers, managers, so they may know that particular person better. -   Ask local charities for support or advice on particular medical conditions or aids that could be used to support the individual Outcome 4  –   Understand confidentially in adult social care settings   4.1. Define the term confidentially -The word confidentiality means to keep sensitive and personal information about the individual out of public view and not to be discussed with anyone other than necessary and needed. 4.3. Describe situations where information’s  normally considered being confidential might need to be shared with others Situations where information normally considered being confidential might need to be passed on is if an individual needed medical care, they may need to know medical background, medication and personal stuff about the individuals. Introducing new staff members to the team, as they will need to know thoroughly certain things what not to speak about in front of clients that may upset them. Or allergies and medication. 4.4. Explain how and when to seek advice about confidentially When to seek advice about confidentiality is when you are unsure about a topic in question, and to seek advice, ask any senior member of staff you think could help. For example, a manager or team leader.
