   Region VIII (Eastern Visayas) Division of Leyte InopacanDistrict GUADALUPE ELEMENTARY SCHOOL OBJECTIVE RECORD SHEET For September, 2014 I. OBJECTIVES:( SMART   ) A. INSTRUCTIONAL LEADERSHIP (30%) 1.   Shall check daily plans of teachers 2.   Shall observe at least 10 classes in K TO 12 Healthclasses and Science classes 3.   Shall conduct post observation of teachers being observed. 4.   Shall conduct informal visits to all Grade 5 & 6 classes 5.   Shall observe the provision of differentiated instruction to address needs ,abilities and interest of all types of learners 6.   Shall monitor the reading level of pupilse,g PHIL. IRI 7.   Shall monitor the mathematical ability of Grade 3 pupils 8.   Shall participate in academic and non-academic competition in the district: e.gScience Quiz Bee, YES O Camp ,HEKASI and Math ,DAMATH and Sports Competition 9.   Shall encourage pupils to continuously register Boy and Girl Scout of the Philippines 10.   Shall monitor the utilization of TG’S & LM’s in Grade 1 , 2& 3 11.   Shall utilize summative assessment (e.g ,by using unit or grading periodtests) in order to give information to th e teachers of his/her learners’  performance in relation to the entire class( Performance Assessment ) B.LEARNING ENVIRONMENT( 20 %) 1.   Shall create an environment that promotes fairness among pupils ,teachers in appreciating one’s achievement in their own diverse capacities and resources  2.   Shall monitor the provision of interventions to address learning problems of the students or set up special programs for the development of special abilities and interests 3.   Shall monitor anti-bullying policy 4.   Shall monitor GulayansaPaaralan Para sa Feeding Program Implementation. 5.   Shall plant at least 10 malunggay trees per month C.HUMAN RESOURCE MANAGEMENT AND DEVELOPMENT ( 25 %) 1. Shall conduct Training Seminar on Dengue Fever Prevention and control with approved training design and proper documentation 2.   Shall supervise participation of at least 75% identified stakeholders on the Training Seminar on Dengue Fever Prevention and Control 3.   Shall observe 100% attendance in all school/district related activities 4.   Shall encourage at least 1 teacher to enroll in graduate studies 5.   Shall conduct staff conference once a month 6.   Shall encourage teachers to conduct action research D. PARENTS INVOLVEMENT AND COMMUNITY PARTNERSHIP ( 15 %)  1.Shall conduct advocacy on Dengue Fever Prevention and Control 2. Shall conduct PTA meeting for information drive on the conduct of School  –  Based Feeding Program 2. Shall involve at least 75% of the parent beneficiaries to support School- Based Feeding Program 3. Shall involve at least 75% of the PTA to support GULAYAN sa Paaralan Program  4. Shall establish smooth and harmonious relationship between and among teachers, students, families and the community at large to work together in bringing about better and improved school performance. (Linkage Mgt.) 5. Shall encourage stakeholder to support pupils would be PARDO’s  6. Shall coordinate with the Master Teachers in identifying problems of the school E. SCHOOL LEADERSHIP MANAGEMENT AND OPERATIONS ( 10 %) 1. Shall manage MOOE judiciously to improve Academic and Physical Facilities 2. Shall submit accurate financial liquidation on time 3. Shall prepare /submit all accurate required reports    ORS    MAR    MSR with M FORM 3 b-1    M form 3B- 1 and Observation Sheets    Form 6, 7, 48 4. Shall craft workable and comprehensive SIP,AIP 5. Shall check/monitor teachers/Pupils Absences 6.Shall identify Pupil at Risk of Dropping Out (PARDO) of the school 7.
