Dermatitis herpetiformis, also known as DH and Duhring’s disease , is a chronic skin condition caused by a reaction to gluten ingestion.   Gluten Ingestion. Gluten  is a family of proteins found in grains like wheat, rye, spelt and barley.  Of the gluten -containing grains, wheat is by far the most commonly consumed. The two main proteins in gluten are glutenin and gliadin. Gliadin is responsible for most of the negative health effects . Signs and Symptoms of Gluten Intolerance.  - Bloating. Dermatitis Herpetiformis.   Dermatitis herpetiformis  was first described by Louis Adolphus Duhring in 1884. A connection between DH and Coeliac disease was recognized in 1967 ,   although the exact causal mechanism is not known.  Dermatitis herpetiformis  ( DH ) is a chronic autoimmune blistering skin condition, characterised by blisters filled with a watery fluid that is intensely itchy. DH is a cutaneous manifestation of  Coeliac disease. Diagnosing DH with Skin Biopsy and Blood Tests. Skin biopsy is used to confirm a diagnosis of DH. Dermatologists usually use what’s called a “punch biopsy ” to remove the skin and test it for dermatitis herpetiformis. After injecting a local anesthetic, your dermatologist will use a tiny, cookie-cutter-like punch to remove a 4mm sample of skin.  The incision can be closed with one stitch, and generally heals with very little scarring.   Blood tests for other antibodies commonly found in people with celiac disease  –  antiendomysial and anti-tissue transglutaminase antibodies  –   supplement the diagnostic process. If the antibody tests are positive and the skin biopsy has the typical findings of DH, patients do not need an intestinal biopsy to confirm the diagnosis of celiac disease. Signs and Symptoms of Dermatitis Herpetiformis. Dermatitis herpetiformis is characterized by intensely itchy, chronic papulovesicular eruptions, usually distributed symmetrically on extensor surfaces (buttocks, back of neck, scalp, elbows, knees, back, hairline, groin, or face ). The blisters vary in size from very small up to 1 cm across. The condition is extremely itchy, and the desire to scratch may be overwhelming.   The signs and symptoms of DH typically appear around 30 to 40 years of age, although all ages may be affected. Prevention of Dermatitis Herpetiformis. You may be able to prevent complications by avoiding foods that contain gluten. Although difficult, sticking to a gluten-free diet can reduce the amount of medicines needed to manage the disease . The symptoms of DH may go away if you cut all gluten from your diet.
