A majority o the lung cancers present as a result o theinvestigation o some new respiratory symptom or because thepre-existing respiratory state o the patients has worsened [1].    A small percentage o the patients present with no symptoms, ewpresent with non-specic symptoms o malignancy and lastly, ewpresent with symptoms o metastasis.Spontaneous pneumothorax is generally attributed to a rupture o the sub-pleural blebs or the emphysematous bullae [1]. This cancomplicate the primary or the secondary lung tumours. Spon-taneous pneumothorax as a presenting maniestation in primarylung tumour or lung metastasis is very rare. We are reportinghere, a case o lung cancer with spontaneous pneumothorax as apresenting maniestation. Case RepoRt  A 65-year aged armer was suering rom cough with scantymucoid expectoration, dull aching chest pain, anorexia and weightloss or 4 months. There also was a presence o haemoptysis on3 occasions during this period, which subsided on conservativetreatment with oral antibiotics. Suddenly, on a day when he wasworking on his eld, there was a bout o cough ollowed by acuteshortness o breath, or which he was admitted to our department.He was a beedi smoker or 50 years, with a smoking index o 1000.He had no past history o any anti-tubercular drug intake or anyother signicant past medical or surgical history. On admission, hewas ound to have MMRC grade IV dyspnoea with cyanosis and ahyper-resonant note on percussion on the let side o the chest, withmediastinal shiting to the opposite side. An urgent chest X-ray wasdone, which revealed a let sided pneumothorax and compensatoryhyperinfation o the right lung, with a transmediastinal herniation tothe let side [Table/Fig-1(A)]. Immediately, an intercostal chest drainwas put on the let side in the 4th intercostal space, with waterseal drainage. The sputum smears which were made to check or Acid Fast Bacilli (AFB) and malignant cells, both were negative    O  n  c  o   l  o  g  y   S  e  c   t   i  o  n KauShiK Saha, Debabrata Saha, arnab Maji, Debraj jaSh, arPita banerjee Ky Wods:  Pneumothorax, Spontaneous, Lung cancer  aBstRaCt Spontaneous Pneumothorax (SP) is usually attributed tothe rupture o the sub-pleural blebs or bullae. Spontaneouspneumothorax is rarely seen in association with lung cancer.Pneumothorax can be the frst sign o lung cancer. The mostcommon possibility or SP complicating lung cancer is thetumour necrosis mechanism or, in separate cases, the ruptureo the emphysematous bullae. Lung cancer should always beconsidered as a possible cause o SP in elderly patients or inheavy smokers. We are reporting here a case o central bronchialcarcinoma with spontaneous pneumothorax, as it is a very rarecomplication o primary bronchogenic carcinoma. or 4 consecutive days. On repeat chest X-ray, the presence o a let sided collapse with a rounded homogenous opacity wasobserved. A CT scan o the thorax with contrast revealed let upperlobe collapse with a fuid lled cavity in the let lower lobe, whichwas surrounded by consolidation and compensatory hyperinfationo the right lung, with a transmediastinal herniation to the let side[Table/Fig-1(B)]. There was no pneumothorax ater putting theintercostal chest tube on the CT scan o the thorax. On spirometry,the presence o irreversible moderate obstructive airway diseasewithout reversibility the post-bronchodilator pulmonary unctionparameters were FEV  1  /FVC-68%, FEV  1 -72% and FVC 80%) wasobserved. Fiber Optic Bronchoscopy (FOB) showed a caulifowerlike reddish growth occluding the let main bronchus within 2 cmo the carina [Table/Fig-2]. Bronchial brush cytology demonstratedendobronchial cells along with infammatory cells, metaplasticsquamous cells, histiocytes, red blood corpuscles and a ewdiscretely lying squamous cells with pleomorphic hyperchromaticnuclei. A biopsy rom the growth revealed an inltrating tumourwhich was composed o large epithelial cells with ocal cellularkeratinization, necrosis and stromal desmoplasia with severe [tbl/Fig-1]: Chest X-ray PA view (A) showing let sided pneumothoraxand CT scan thorax with contrast (B) showing let upper lobe collapse withlower lobe fuid lled cavity with consolidation o surrounding parenchymaand compensatory hyperinfation o right lung with transmediastinalherniation to let side.
